- Please include names and date of birth of all family members who will be attending the consultation in case they need to be examined. This will ensure that the family receive the maximum Medicare rebate.
- Please include all relevant investigation reports with the referral.
- For referrals for a chromosomal microarray anomaly, please forward a copy of the array report. For paediatric cases, if possible, please arrange parental testing prior to the consultation. If you foresee issues with this, please contact us prior to making the referral.
